Our Health Information Management Services (HIMS) department maintains medical records for each patient treated at Mountain Vista Medical Center.

To obtain copies of your medical records, please contact the HIMS department at 480-358-6400.

Prior to receiving copies of your records, you will be asked to complete a consent form authorizing release of your information and to provide proof of your identification. 

Requests for medical records generally take up to 72 hours to process.
